My wife is a 90 pound Japanese girl and an A seems to small and a B too big. The American sizes and Japanese sizes are not the same, so she has no idea what size she wears.

Here are the sizes in order of smallest to largest:
AA
A
B
C
D
DD
AA is the smallest, and that means that your bust measurement is less than four inches bigger than your rib measurement.

No it's smaller than an A cup. It would go AA, A. She's not quite an A cup
Take her to Victorias Secret or a lingerie department and have them measure her for the right bra. Maybe the band is too small? Maybe she needs a 34A vs a 32A?

No, it is not between an A cup and a B cup. It is smaller than an A, the smallest bra size possible. For example, jeans -- 00 (Double Zero), 0, 1.... It's just like a double zero, except it's a bra and a double A.
