No it doesn't need to be amputated. The only time the amputate a foot is if there is no circulation to the foot. So long as there is circulation to the rest of the foot it wouldn't need to be answered.

Not necessarily. Why are the toes gone? If it was due to circulation problems then it may be necessary in the future, but it simply depends on whether the rest of the foot gets enough circulation to keep it viable.

Just amputate from the neck down and there won't be any more problems

take a regular fin and stuff a sock or something like that into the toes , may need to adjust till comfy
