I'm still in high school, and I'm using one of my teacher as my reference. When I put her in my application form, do I put:
Ms. hername? or
Firstname lastname?
does it matter? (I don't know her first name though...)

I don't think it matters a lot unless there is more than one Mrs. Herlastname. But it is polite to ask her if she would mind being a reference for you before you put her name down. At that time, you could ask her what her first name is.

You can Put Ms. Smith, ABC High School, subject taught, phone number of school. They application may ask for address, be sure to give HS address. The future employer may want to write a letter to your teacher
