It's been about 2 days since the hammer fell on my second toe, my toe now is 100% dark purple black and it's surrounding the skin. I've been putting my feet in warm water, cleaning it with alcohol and putting neosporin antibiotic ointment. If I go to the ER now are they just going to pull out my nail ? Should I just continue doing what I am doing and just wait until the nail fall off ? It does hurts but I know its from the pressure of the brusie clogged up inside....

There are treatments apart from removing the nail, but even that would be better than the pain you have now. You should also be X rayed for a fracture...;

If there is distinct pain and blueness under the toenail itself drilling a hole is too late; the entire toenail would have to be removed surgically. The blood has already clotted under the nail plate. In regard to the blue discoloration of the toe, the soft tissues are severely bruised; if you have significant swelling of the toe this is indicative of a fracture of the bone or bones of the toe. Placing your toe or foot in warm water right after the injury is inappropriate and counter-productive; the warm water has permitted the toe to swell even further. Following an injury ice applied to the injured part, off and on during the day, is the proper medical care. Infection is your main conern at this point. Increasing pain and swelling of the toe are your key symptoms at this time which would indicate a soft tissue infection necessarily requiring an oral antibiotic.

so what's the question at the end of the story? I don't think going to the ER would do much of anything except charge you money. Keep it bandaged and clean and it will eventually heal itself. If it gets to where it hurts too bad, it could be infected, so go to your normal doc and he'll give you an antibiotic or something.

Go to the doctor, there is a way they put a hole in the toenail and let the blood out. This will relieve the pain immediately.
