need to figure out the amount of wire you need, are you planning on going inder the floor? through the attic? then the rise/drop needed and the location you want to tap your existing phone line. easiest is at the box outside your house.
items needed - wire (4 wire phone line red/green/yellow/black)
wall mount
cover plate.

tap into the phone box outside your house and run phone wire wires to where ever you want your jack to be. the wires are very small and color coated. and there are multiple wires with it.
