If they know you are working- they should understand that you would like to give your current emplyer two weeks notice. So once they offer you the position- tell them that you will start two weeks after you accept. Sometimes they may take 6 weeks just to hire you, so they should understand...

I wouldn't, just write down the next-to last job you've had. The only time I've written down jobs I was fired from when I was just starting out and needed the experience on my app. I always told my employer not to contact that former employer because there were issues with "insert random issue here". The company isn't going to contact the FBI and find out everything you've ever done before hiring you. Just put any work relevant to your field and make sure everything is consistent!

two weeks from the date of acceptance. You must also give your current employer 2 weeks notice of intention to resign.

If you want to use your current employer as a reference EVER, then 2 weeks notice is standard. If you don't care about them & will never use them as a reference, then you can start immediately.
