If you contact the company for whom the model is working, they may be able to tell you the name of the advertising agency that does the ads for them.
By contacting the ad agency, they may be able to refer you to the model's agent. The agent may know the specific magazine ad that you are referring to. It probably won't be easy, especially if you are just a fan who wants to write a fan letter or compliment the model.
It'll be a challenge.

They usually identify the cover model in the magazine's table of contents.

ussually in the summmary of the magazine (can be anywhere from 3-6 pages in) will have everything in the mag plus the people that are on the cover.
