Wow, that's a lot of question. Well, here goes :
1. Actually, it still not very clear. Itachi said that it's all for testing his ability (in japan, he said utsuwa, means vessel or container). Although he also briefly makes reference to some ultimate purpose behind the Uchiha clan, but what this is has yet to be revealed.
2. Sarutobi (I don't know the clan name)
3. I don't know. I don't even remembered that it ever mentioned in the anime or manga. Sarutobi even calls him as Fourth. Fourth nickname is The Leaf's Yellow Flash or Konoha no Kiroi Senko. He's Naruto's father, isn't he? So at least his clan name is Uzumaki.

1) The leader of Akatsuki was planning on killing the clan himself but instead Itachi begged not to kill Sasuke, So The leader told Itachi to Kill them instead and spear Sasuke. (have you noticed, Itachi never killed anyone ever since he arrived to Konoha, but he injured some but still it means something)
