I usually just wet the old piece of soap and the new bar of soap and then squish them together in the shower so they stick to each other.

I don't have a recipe for putting all the little pieces together, but I think that might be a little intensive, it would take a lot of little pieces to make a normal sized bar.
Instead, I've heard it's a good idea to put the pieces in a small cotton sock, so it kind of doubles as a washcloth. The little pieces you may have will be used up within one or two showers anyway, and then you just wash the sock for next time.

I take my pieces and soften them up in the microwave. I don't melt them completely because they get too hot to work with. I then scoop them out of the bowl with a spoon and place on saran wrap and mold them into the shape I want. You can add a little water to smooth it out a little better. They harden right away and work very well.

You can get a plastic thingy from Wal-Mart that takes the soap and you squeeze it into one bar, we haven't tried it and are looking for it. They've been out.

My grandma used to put them into a old nylon stocking, and tie it into a knot and cut off the excess and use it for cleaning the tub. It works real well.
good luck!
