You mean sitting out at room temperature? It's OK at room temp (66-72°F) for up to 10 hours. In a warmer room, 4-6 hours.

I've read that it can be kept at room temp for 4 hours, in fridge for 48 hours and frozen for 3 months.

What do you mean leave it out? You can't just leave it out in the open for too long. Keep it refrigerated or frozen. If you freeze it, thaw it by running hot water over it, if in the fridge, warm it by setting it in a bowl of warm water. Test it on your wrist. If it starts to look miscolored or have a slimey looking substance on the top, you got it too hot, those are called hotspots and will burn your babies throat. I hope this helps.
