No kidding. I'm embarrassed to admit that it was until about 5 years ago - after having owned the album for I don't know how long - that I FINALLY realized what the song is about!
I'm not a "smoker" so I guess that helps explain it.
I just thought it was an unusual nickname for a girlfriend.
