i have lost mine and need it as proof of i.d.

hiya!! just dealt with the same problem at home...if you contact birth deaths and marriages,for a small fee you can get a copy of your original birth certificate!! good luck

You need to go to the city offices for the city in which you were born. For some cities, it is actually obtained from the county offices. You should call the city offices first. You will need to bring some form of identification. The cost is usually about $12. If you are under 18, one of your parents will have to obtain it for you. If you are over 18, only you can obtain the birth certificate.

Either write or go online to the state where you were born. Ask the State Statistics Department to send you a form to fill out and mail back usually with a $10 fee and they will send you a replacement birth certificate.

yes a birth certificate obtained from vital statistics or another government facility will have a seal on it. Most of the time its an embossed seal, meaning the seal has been embedded into the paper. Also, when you send in the birth cert. for your passport you need to send the original copy and not a photo copy. They will not accept a photo copy because they cant see the seal. The passport office will return your original birth cert. back to you with your passport

You don't need to fill a form in, just write to your local office, give Mum's name, Dad's name, your name and date and place of birth. Costs about £7 in the uk and they will send you one by post. x

all you need to do is go to your local registary office and apply 4 1 there, i had to do that when i lost mine it takes 5 mins to fill a form out costs 7.50 or therabouts and you get it sent to u in the post
