If one of the person's ears is facing you, then you can look at the back of their head AND see if they're smiling. Or you can feel their face while standing behind them (not intended with strangers). If you're allowed to use an object, the other person can look into a mirror while you stay unfocused near the area of the back of their head.

No, it would be impossible to see if someone was smiling by looking at the back of their head unless of course you asked them, were informed by someone else or perhaps if you were both loking in a mirror.
