The lens in your eye may be too strong and thus focus the image of the outside world in front of your retina, or it may be to weak and focus the image behind the retina. By placing a lens (glasses) in front of the eye, we can compensate for this by ensuring the image falls exactly on the retina, which allows for good vision.

There are several types of glasses available, in several corrective prescriptions.

In some cases, the correction is required due to an abnormality in the eye structure (wrong lens shape, poor muscle control, lack of protective cornea). In other cases, it is to compensate for a physical/chemical imbalance in the nerve-brain connection (using polarized lenses because the retina can't effectively block out the harmful sun's rays). Finally, there is the case where glasses are required by law to protect eyesight from foreign objects (safety lenses and side shields).

it is because the vision is unsharp.
If you look through a magnifying glass you will notice what it is unsharp.
Negative lenses are used to correct the far vision, positive lenses are used to correct nearby vision ( reading )

yes, your guess is correct. Lens goes bad b/c of medical conditions, or using it only for looking at close objects (TV, computer, book)
