Simply put..
There are more rules in American football than any other professional sport.. You almost need a PhD to understand every rule.. That's the first reason..
Second is passion as a kid.. you becoime passionate about the sports you are exposed to as a youth.. every kid in the united states that went to high school knows someone who played high school football.. your school and your city pride was always at stake every Friday night..
By the time you leave high school, it becomes your university and your state's pride in college..
That doesn;t exist with American football any where else in the world..
Soccer, on the other hand, it's very similar.. every town, every school every region in most countries have teams that represent in the same fashion as high school and college football do here..
But take the vast majority of the life long soccer fans from any region of the world and introduce them to football, most.. not all, but most will find the game very confusing, difficult to follow and the number one question which I hear from so many Euro and South American friends is why is it called football when they only use their foot less than 25% of the time.,.
Bottom line is what do you grow up with.. People in Georgia or Mississippi have a hard time following the NHL because they have no connection.. the same, to a lesser extent, to the people in Wisconson, or Minnesota with NASCAR.. Football is widely followed here in the US.. but most Germans have no clue what the difference between a forward pass and an audible is

It has... sort of. There is the NFL Europe and the CFL which bring football outside of the USA. There was a NFL game played in Aztec Stadium in Mexico between the Cardinals and the 49ers. There has been an deal signed that will play some preseason and regular season games in other countries like Japan and Mexico.

I love it, but most non-North American sports fans seem to see it as a bunch of fat guys tackling each other. I don't think they're right, but that seems to be the attitude, and that will be hard to overcome.
I'm also not sure that the NFL has really tried to market their game in the same way that MLB and the NBA have done. Both of those organizations seem to have a much large worldview than the NFL, although both need it because they can't touch the NFL's popularity in North America.

It is a slow, boring game with too many breaks. Wimpy fair catches, three time outs. Snooze fest. CFL is much more exciting than NFL. Soccer has no breaks until the half and no time outs. Constant action. More skill is needed in soccer.

Because you can't be a wuss and play football. That pretty much leaves out every country except the USA.
