Usually hospitals in India would not hand over the case-sheet/case file, but they would give you what is known as a 'discharge summary'.

A discharge summary is one which says when you were admitted and with what problems, and who were the consultants attending on you, the investigations done and the treatment given.

A discharge summary, in short, is almost your case file and is valid in any court of law in India. You can go and ask for a discharge summary if you roughly remember the date/month you were admitted and see to it that the summary is signed by the consultant attending on you or atleast by the duty medical officer.

You have to go to that hospital to retrieve your file.
