I would offer the following pointers:
1. Treat your mind like a muscle. Body builders work their muscles very hard for awhile, then give them a two- or three-day rest. Your brain needs the very hard work (study) and it also needs the rest. Just as body builders will work a different muscle group every day, you may find that you can study a different subject after your mind has had its workout with the first one.
2. Be sure and rest well the night before the exam.
3. Get a little exercise just before the test, especially if you are feeling nervous.
4. Think of the test as an opportunity--as a challenge. It's a chance to prove how well you know your stuff! (Being nervous or afraid of the test will not help.) Think positively!
5. If you are daring enough, and can "stomach" it--try fasting. Going to the test on an empty stomach can free up some blood-supply for the brain. The downside of this is that, while you may be able to think more clearly, you may find the stomach growl distracting. (Water is fine, though. You don't want to be dehydrated.)
6. At a minimum, eat lightly, and not of rich or spicy foods, prior to the exam. (Vegetarians have an advantage here.)
7. A relationship with God will provide great moral support. Trusting to Him to help you, and praying before the test, can really do wonders to alleviate negative stresses and emotions, and free you up to do your best.

Here's what I would do: whenever you get notes in a subject study them that night so when the exam comes you don't have to study so much the night before, and do your homework; practice makes perfect, and when in doubt on a "multiple-choice" question always go with the one you thought was right first unless of course you know it wasn't right.

I have one study night every 2 weeks. On this night, sit at your desk, open your books and take down notes into a notebook on the subject the exam is in. Then you should read over your notes three times. On the day of your exam, you should read over these notes twice and that way, they will still be fresh in your mind.

review notes and information the day you get it.
make sure you get enough food/drink/sleep.
chew mints for concentration.
drink a glass of water AS SOON as you wake up (for alertness).
listen closely to every word the teacher says.
take notes.
DON'T PANIC!! (that will only make you more nervous...)
learn from previous mistakes on earlier tests.
