I'm not sure if this will help you, but my cousin's oldest daughter has this same problem. SHe is 14, though, and is being seen by specialists at a special hospital. She is 3 feet tall. It is her thyroid and they are trying experimental medication on her. She may have grown an inch in the last year, which is great. I also have small hands, but not smaller than average. I would take the advice and get your thyroid tested. Then again, due to that hormone missing, that may be the reason behind it. Is it Estrogen?

It's highly unusual to have never had a period by age 20. More than highly unusual. If you've had brain scans and such, you probably know this, and you're not going to find the answer on Yahoo Answers, though I don't blame you for trying.

Menstruation is a hormonal-related issue. Do you consider the rest of your physical development to be normal? Specifically, have you grown and matured sexually (physically, like breasts) similar to other girls your age?

The only other thing I would note is, do you consider yourself to be overly thin or extremely athletic? It is not uncommon for women who are extremely thin, or anorexic, or just extreme atheletes to fail to menstruate. If you've been that way since you were 12 or so, it's possible that you simply never started at all.

My only concern about this guess is that before a brain scan, someone should have pointed that out first. But if not, that's the best I can do.

If you have honestly had a pap test your doctor should know what is wrong. Not having a period by 20 is not normal at all. It could be to several different things though. I would go to a gyno and get checked out fast. It could be somthing serious or somthing very small.

Have you had a chromosome analysis done. It is rare, but you could have Turner's Syndrome, where you are missing one of your X chromosomes (XO instead of XX), or you could be a mosaic for XO (XO/XX). Women with Turner's can be outwardly normal, and in fact are quite healthy, but may have delayed or absent puberty.

Any hospital lab with a cytogenetics department can look for this.

Good luck.
