Now, the person who HIRED me, was the owner. And he never really supervised me much. I only met him... 4 times?

So, do I put someone else? Like the manager? (Who supervised me and evaluated me...) Or the person who hired me?

They want the name of the person who oversaw your work - in your case, the one who wrote your evaluations was the manager, so use that contact.

You can give reference to both teh Hiring Manager who is the owner and your immediate boss(Manager) who graded you for your work.
Generally its a good idea to give references to both as they have evaluated you differently and with a different perspective.
For eg. Your manager has evaluated you on day today job, your performance etc. Whereas when u were hired the Owner saw something in you that he thought would be good for the org and hence hired u for that.

Your best bet would be to put the Manager who evaluated you. This person can give specifics about your work performance.

The person who hired you might have had authority to make hiring decisions.

You should put the person who actually saw your work and could advise the potential employer about your strengths and weaknesses....although, these days many employers will only allow salary and dates of employment information to be shared. good luck with your job search.

The supervisor's name should be the person you directly reported to. In other words, who was your actual boss?

I would put your manager's name. It sounds like he is your immediate supervisor.
