He needs a kidney function test as well as checking for any blockage in the ureters.

Otherwise:
How many times does he pee each day? Measure his output in the morning after sleeping all night and again after holding it for 8 - 10 hours from morning to late afternoon? The morning pee will tell his bladder size: 1 pint (500 ml)--normal. 1 quart (1000 ml)--a large bladder. A quart and a pint (1500 ml)--a mega bladder. 2 quarts (2000 ml)--one of the world's largest. Ready for a freak show.

Some people who have very large bladders don't have a compelling nerve feeling in their bladder until they are bursting. A beer is a minor diuretic as are, in order of strength, coffee, tea, Pepsi, Coke, and other caffeine drinks. They tickle the bladder's nerve endings and prime the pump. He ought to rejoice at being able to hold that long without pain. I am envious.
