A dream has nothing to do with wants going on in your life, UNLESS,whats going on in your life today is your sub-con-consciences. Your mind can help your body to deal with the tension of over load. You need to know the body always needs to ex-spelled. I think when you have a lot on your mind, like your grandmother being taken ill, it has an effect on your dreams.
Your brain is probably thinking lots of things through and this fear of losing someone, albeit your mum or grand-mum, is upsetting you.
I would say from my own experience of seeing a friend/relatives die in dreams, that it is little more than your brain working overtime, and homing in on a few fears or things that would mean a significant change for you.

Death is change. It is more important to see how you feel about your dream than just the meaning of seeing your mum die.
Your grandmother is your mother is you. Just seems like seperate people but you are all the same lineage.
You are aware that this is something that is undergoing change.
