Put me down on the list of people saying "take it."
If your boss feels your ready, then go with it. It's your bosses job to help you succeed. The might mean hand-holding, training, coaching, whatever.
Also, was the person who had that job the one and only best in the world? Unlikely (especially of that person didn't groom self confident next-in-line direct reports).
Here's what'll happen. You say no, your boss goes to the person next in line who is, in your bosses mind, less qualified than you, and offers that person the job. Then you'll be working for an idiot who you'll resent for
1- being an idiot and less qualified than you
2-getting paid more to do less--which would've been your job.
Take the offer. Don't over think it.

I would tell your boss, that you are flattered but you don't think you could excel as a supervisor at this time due to life situations at this time. And you don't want to disappoint you and would never do something You don't thing you could give every thing too. Please consider you in the future.
(most likely you will close promotions offers if you do this)

Bad move turning it down. You basically have to take it. I've heard of people being canned for not stepping up and taking a promotion. If there are certain things that you can't handle, let him know your concerns about those things, but don't turn it down flat out.

Let your boss know how you feel. Let him/her know that you still want to stay in the position that you are into right now and are not yet prepared for a bigger responsibility

once you turn down a up grade promotionn you will never be considered for a promoted again.
take that promotion or leave in a few year to get a better job

Your boss seems to think that you are ready for it (they are usually right about these things)...Why do you think that you're not? Maybe you are just scared of change and more responsibilities.
