Don’t know the origin of the phrase but it does mean it’s time to start drinking. “Sun’s over the yard arm” and “first one in the left hand today” are a couple of others. Back in the 70’s a friend had an analog clock in his bar that had 5's on all 12 number spots, I think it ran counterclockwise too.
Now that I think about it may have started with “Five O'Clock World by The Vogues” a song from the mid 60’s about the end of the work day and time to have fun.

It's Alan Jackson. The song is about drinking early in the day instead of waiting untill after work ( 5 o'clock it the traditional quitting time) So if it's only noon you can get away with drinking by saying, it's 5 o'clock somewhere.

First time I heard it was in Dick Tracy, Madonna's character said it to Dick Tracy when he said he got off work at 5pm!
But I think it means, with regard to cocktail hour; i.e., you can have a cocktail anytime; "it's a big world, it must be 5:00 somewhere".
