Even if birth control is used perfectly it will occasionally fail. About 3/1000 women who use the pill perfectly will still get pregnant within a year. It's always a crapshoot, and you could have had bad luck.

Now, in typical use, about 8/100 women will get pregnant on the pill within a year. That's a lot bigger odds. Any of these things that you did could have lowered the effectiveness of your birth control pill.

-Not taking it every day at the same time.
-Taking them out of the correct order
-Taking the antibiotic rifampin
-Taking some oral (by mouth) yeast infection meds/antifungals
-Taking some HIV/anti-seizure medications
-Taking St. John's Wort
-Not using backup protection if you have vomiting or diarrhea

Not knowing you I can't say what went wrong, if anything. But any of these things can do it.
