No. Cardcaptor Sakura isn't a continuation of Tsubasa Chronicles.
CLAMP- the creators of these two popular anime/manga series brought characters froma ll over thier previous works.
For example-
Syaoran, Sakura, Toya, Yukito, and Kero originally comes from CCS
Fai and Chi are from Chobbits
Princess Tomoyo is the Tomoyo from CCS
Ashura and Yasha and Arasha are from RG Veda
Yukko the Dimensional Witch and Watanuki are from xxxHolic
Chun-Hyang is from a CLAMP produced manga called The Legend of Chun Hyang her mother is also from this book
Princess Emeraude and Mokona are from MAgic Knight Rayearth...
Clow Reed was first in CCS
As the Dimensional Witch said, there are many different worlds with the same person and thier soul. Except, they live in a whole new dimension. If you are confused about this watch Tsubasa Chronicles Episode 2 again. They explain it thouroughly

I hope that one of the episodes in Tsubasa Chronicles would be that the gang meet the CCS's version of Sakura, Syaoran, etc...

Hope this wasn't too confusing

No, although its partner anime/manga xxxholic does make occasional references to Sakura Card Captor. I enjoyed the Tsubasa Chronicles, though I warn you that it is essentially episode after episode of "knight fighting in his quest to save his princess by being really selfless." Some people find that irritating, and some people really enjoy it. The real meat of the story comes faster in the manga, but you can watch the OVAs of the Tsubasa Chronicles as well. The story drops abruptly darker at that point, with a great deal more intrigue and webs of wishes, which is pretty neat.

Having an entirely different plot, it is not the sequel; TRC just features the very same characters (not only from Card Captor Sakura, but also from other Clamp works).

to take away your perplexity, i would just like to say that tsubasa chronicles is NOT i repeat NOt the continuation of card captor sakura.. the characters there just have the same names and looks of that particular anime because both have the same makers ... they -the makers i mean- probably never got over the card captor sakura series since it was such a big hit.. hehe..

it isnt a continuation, just a crossover series that uses past characters in CLAMP's work to create an entirely new adventure.
