The difference between a "lady" and a "girl" has utterly nothing to do with age as many of these answers are indicating. It has everything to do with “feminine integrity”. I know a lot of older women who are still "girls" and have never embraced the totality of their feminine integrity. Yet, simultaneously, I know a few younger women, who are truly "ladies" having a feminine poise and demeanor of great integrity.
A woman with a “girl” mentality possess the ghastly attributes of being immature, crass, harsh, barbaric, rude, lacking in social graces, caustic, catty, mentally childish, primitive, loud, pushy, temperamental, back-biting, bossy, brash, conceited, crude, disrespectful, tacky, whorish, negative, selfish and uncivilized.
A woman with a “lady” disposition possess the refined attributes of being graceful, delicate, purposeful, prudent, soft-spoken, charitable, chastity, reserved, thoughtful, mentally mature, sophisticated, polite, hospitable, considerate, caring, respectful, kind, good-humored, modest, positive, selfless and civilized.
It is a modern cultural travesty that very few women today can claim the attributes of a “lady”, having high standards of feminine integrity. I put the entire blame at the foot of the "Feminist Movement". Feminists in their effort to be equal to men have, instead, become "like" men, defeminized and masculinized, literally transforming into that which they hate, men, the price of misanthropy. The “Feminine Mystic” has become the “Feminine Mistake”, resulting in the death of femininity. Women are completely out of touch with their True Femininity and the “power” that their feminine integrity once welded. These now lost attributes that once made women the “Great Civilizers” of society are no more. Because women no longer operate in these attributes and have become in a sense “cave women”, the world at large is becoming more barbaric, for the former presence of the “lady” once tamed the aggressive nature of her male counterpart with gentlemanly reserve, but now it is entirely absent. Hence, the world is progressing towards uncivilized dystopia, a “Mad Max Over Thunderdome Syndrome”.

Age-maturity level. A lady knows where shes going in her life what she wants etc. A girl however is still naive and not sure of where her life is going

I think of girls as 'pre-pubescent'. 'Young women' up to eighteen. Then I suppose it's 'woman'. When referring to a group of women, it's usually friendlier to call them 'the girls.' However, if you are dealing with business/feminist types refer to the PC code of behaviour. I think that the word 'lady' is equivalent to the male 'gentlemen'. It tends to be used in more formal situations, or perhaps, cultured types. The word 'lady' could replace 'woman' as used above.

Girls are immature, narcisisstic, selfish, hypocritical, confused, brats. Ladies are mature, sensible, caring, giving, emotional in a rational sense, and fair.

A "lady" usually wears a red dress, black stilletoes, has a really tight perm and chews gum. A girl is more open to ideas.
