Yes it is a foul if you touch aggressively the other player. Running after you dribble is called a traveling. If you are in the 3 second box and they count you 3 second it is a foul.

1. Is it a foul if the you're on a fast break and you screen the defender running after?
It's an offensive foul, if you fail to set your feet "Before" the defender arrives. It's called "An illegal screen". Coaches disagree with officials often, because it's a judgment call by the referee.
2. Is it a foul if the you're running after your dribbler and you fall over?
I assume you mean, if you're on defense.
It's a defensive foul if you trip the the offensive player when you fall over.
If you're on offense and you trip someone, it's an offensive foul (like an illegal screen).
3. The 3 second inside the key rule, does the 3 sec count reset after each pass or only when you move outside?
You DO NOT get three seconds for each pass.
Players must exit the lane in three seconds, or the offensive team must shoot the ball. You do get another three seconds, if you get an offensive rebound after a missed shot.

about the fast break, if you are standing still, you may not be called for the offensive foul the defender could get the defensive foul instead...but as a player, instead of setting the screen, just fill the lane and get ready for the pass. it would be much better.

about the three second rule, the count resets when you go outside the shaded lane....

question#1- there is no foul on anyone in that screen circumstance. Brad Miller screens people like that all the time and lays them out. I saw someone leave the game after running into miller so hard and no foul was called.

question#2- 3 in the key resets when you leave the key

Nice name...

I'm not sure about the foul because I would have to see it to give my opinion. The refs will probably see it as a foul anyway but refs sux. The 3 second call only gets reset when you move outside the key.

If you set a legitimate pick, it's no foul. But if you and the defender are both running, it's going to be hard to get into a set position.
