1) Kakashi got the Mangekyou Sharigan from Obito (sasuke's cousin). Obito was dying, and so since Kakashi was his best friend, he gave him the sharigan (but only on one eye).
2) No Gaara doesn't die. He becomes all good thanks to Naruto (just like Naruto is able to turn Neji's life around).
3) Sasuke shows up again after the 2.5 years during the manga chapters 306-308.

It is still not revealed yet how he got his Mangekyou Sharingan... (the answers above this are how he got his Sharingan, not his Mangekyou Sharingan)... it can be presumed that he got this during the timeskip...
Yes, Gaara died... but he was later revived... so he lived...
After the timeskip, he first appeared in chapter 306...

kakashi got his sharingan when one of his team members named obito in kakashi's team which was from the uchiha clan it happen when Kakashi lost his eye when he was trying to save Rin and as a result of an enemy ninja attack.Rin was a medical ninja and implanted obito's eye as a gift to him when obito died saving kakashi from the ninja. and if you want to know why kakashi is always late when he has to go to meet up with naruto, sakura and sauske to give them missions is cause he is alwayas visiting and praying for obito in that monument where they have the heros that died in battle.
gaara becomes a kasekage and becomes good friends with naruto.
sauske does appears after the time skip.
