Depends how extensive you want the web form to be and what you want it to do. If you want a shopping cart system, then you'll have to pay someone to have you make it and configure it for your server. But if it's as simple as a few buttons and txt boxes, someone here might just do it for free.

Easiest thing is to just learn some simple HTML yourself.. It will help you if you ever need to make changes to that form.

There are thousands of websites with millions of html scripts already made that are there for free.

If you want a specific web page or website created, you should be thinking in terms of paying a coder to write it for you.
