There are an infinite number of 'forms' available - what type of forms do you require? A good place to start is with a text book of the field of medicine in question. At this time - much to my amazement - virtually every book is available online without charge if you spend a few minutes looking. There are books of forms - books of flow charts - and books of algorithms. If I know what you are seeking I will be of more precise assistance.

Call the insurers and they should send you some forms. http://emory.hr.emory.edu/forms.nsf/0/1063a893a91f1cc285256d010067c55a/$FILE/Aetna%20Claim%20Form2006.pdf
