Normally a doctor will NOT give anything to a 4 month old. It is best to let the cold run its course. If you stop some of the symptoms, such as runny nose, you get sinus drain down the throat, etc.
Just ride it out and let your little one build his immunities.
Be sure if his nose is stuffy that you suction it out with the bulb syringe when he needs it.

Do not give him anything except cool mist humidifier, suctioning out his little nose before and after he eats, a few drops of saline will work, tinny noses is one brand, and his normal feeding. If he has a fever Tylenol every 4 hours as needed for a temp over 100.4 or if he seems uncomfortable. Usually at this age they do not give cough medicine b/c it really does not do anything. You do want to call his doctor if he is unable to breath, meaning is he weasing or retracting. (you see his ribs with every breath, ) is he blue, hot to touch. a dry barking cough or a hoarseness to him. Call his doc anyway let them know your little one is not feeling all that well.

If you go to the chemist they can usually recommend the right type of cold syrup for young babies. I just like to use some lavender oil in a burner in the room that they sleep in and a little bit of eucalyptus oil on a tissue of hankie tucked inside their clothes, but never on their skin as it can cause a rash.

And I'm pretty sure that unless the baby is extremely uncomfortable that your Dr. will say, don't give him anything...
