Within Houston, In person You can make a stop by either the
Harris County Administration County Clerk Office in Downtown Houston on Preston between Fannin and Main,
or at the Houston City Health Department located off Old Spanish Trail between Kirby and Greenbrair not far from the Astrodome/Reliant Park.
If you are not in harris county, you can pick it up at any area county clerk office
