The main reason for involving a doctor is me little mate, will be a friend for life, and when it is not happy, I am not happy, to say the least.
Also, if severe there is a risk that overly tight foreskin can damage any sex partner.
There is a possiblity that there is some sort of infection otherwise unnoticable that is doing some of this, yet another reason to see a Doc. It happened to an uncle of mine. It also doesn't mean you are unhygenic, it can simply happen no matter how hard we try.
The pain in this case would be caused by teeny rippings of tissue, too small to be seen.
Finally because the foreskin is in the process of getting tighter, it is possible that the tighness is due to quite a bit of weight loss, however, I imagine the same treatment will be the same.

Hello mate. Listen, I had the same problem and it turned out to be a simple yeast infection. Thats right I had thrush. You say that you have been going to the gym alot, I dont mean to insult you but do you shower afterwords every time? and if you do, do you/did you wash your old man thuroughly?
The reason I ask this is because a yeast infection can be caused when conditions are hot and sweaty so you may have got into a vicous cycle. Once the infection becomes established you need to treat it but it is not too serious so dont worry too much.
Buy some '2% Canneston with corizone thrush cream', begin to put it around your galnds and froeskin and hopefully you should start to see it heal. if not then see your doc and he will give you a stronger cream.
One note of caution. Do not try to pull your foreskin back too much as you may split it (like a chaped lip) if this happens the infection will get in deeper and be harder to get rid of.
