I am Brasilian from Rio de Janeiro, so I would like to say
Vasco da Gama x Flamengo! But since there are other great Brasilian Derbys, I'd have to go with Boca Juniors x River Plate cause its a match that stops the entire Argentine nation! The only other derby that comes close is the CelticsxRangers cause it involves religion as well... But considering that Argentina is 2x World Champs and Scotland isn't...
I stick with BocaXRiver!
PS:InterXMilan is a good one, but those arent the only dominant teams in Italian football... A Juventus or Roma fan wouldn't give a crap about that game! But everyone in Argentina cares about BocaXRiver! Much like all Scots care about CelticsXRangers!

I'M not sure some of your answers qualify as derby's but the fact that, THE OLD FIRM has its own name would suggest its the greatest. I have experienced the Manchester derby at both grounds, and really it's not that great. Liverpool seems more important to united. The north London's derby is now being challenged by Chelsea. the Madrid derby has always been second to barca's match. But the passion of the Italians the Milan derby gets my vote, no one sits on the fence when they play.

Real Madrid vs F.C Barcelona is the greatest battle of them all. Nothing can compare to what that match represents.

As far as quality of play given derbies tend to be scrappy affairs, I'd vote for the Real-Barca ones (why do you think Real-Atletico is even considered a derby...that is not the one match Real fans clamor to get tix for).
Also, Boca Jrs-River Plate in Argentina are sensational at times.
I find Brit and Italian derbies more a fan's thing as the soccer tends to be brutal.

The major ones I think are Man U-Liverpool and Real Madrid-Barcelona, because they are the most successful teams in their leagues.
Celtic-Rangers are due to religious reasons, and the Italian ones are mainly because they share the same stadium.

The most fierce is the old firm Derby. I went to a Milan Derby though, the one just past and that is the best i would say. Although my team ACMilan got beat. The Rome Derby is also exciting.
