Assuming you're refering to the original novel by Gaston Leroux, it was a natural disfigurement which wasn't really gone into very much. Depending on the novel or movie, he also had acid thrown in his face, his mother took a poison that she got from a gypsy in order to kill the baby or herself while she was pregnant, a fire broke out, and there's even a version in which he is the son of the devil. Aside from the original, the best novel I've read is Phantom by Susan Kay, if you're asking just because this is a subject that interests you, I strongly reccomend it.

In the book he was born with a disfigured face, there is no mention of a fire or of acid. He also is described in the book as being an all-around physically unattractive guy, unlike the way he's portrayed in the most recent movie.

The person who became the phantom, worked for a music publisher. He approached the owner to get his own music published
and was refused . In a fit of rage some copper plates into a vat of etching acid which splashed in his face and caused a fire.

it seems that each version of the story has a different way he got disfigured. I've seen him get acid thrown on him accidently, get burned in a fire. His mother drank poison and he was scarred in the TV movie with Charles Dance and Teri Polo.

In the movie he had acid thrown in his face because the composer turned him down and they got into an argument and the composers receptionist grabbed a silver tray which was filled with acid and threw it into his face, which then created the fire.
