Hi there - at 75 degrees the life cycle if ich is 3 days so you have to keep find a treatment that will allow you to treat every three days - eventually you will come to the end of the cycle - this parasite can only be killed in the free swimming stage of it's life. So you havent failed it's just that you need to get into the sequence of treatments - the life cycle slows if the water is cooler and speeds up if it is higher.

The old fashioned treatment was methlene blue which was very effective in the removal of white spot some of the new treatments are contain it- clown loaches are very prone to white spot and you will find whenever you add new fish to your aquarium your clown loach will break out with it - they are a good way of spotting white spot as they are the first ones to show signs of it.

Of course it is a very serious parasite and a killer if not dealt with properly.
Best wishes

Raise temperature, change the product, add a bit of salt from the fish store. Clown Loaches don't have scales so they're very sensible to ICH, it might die if you don't act quickly.

You need to try a new medicine. I would recommend QuickCure. It's a liquid medicine you add daily for about a week. It has worked for me many times. Just make sure your water quality stays good or you'll stress the fish out - do water tests and water changes as needed.

The cyst might be too big to treat it, you may have to wait until it bursts before you treat your fish again. For added safety, you can remove plants from the aquarium the fish is in.
