The only legit movie that showed a pretty explicit sex act that I have seen is "The Brown Bunny". Chloe Sevigny performs oral sex on Vincent Gallo. I don't know if it is really his appendage, but she is performing for real on somebody.

Not usually. It can look pretty graphic, but that's thanks to tricky editing. There have been a couple of European movies in recent years where there was a big deal made out of the fact that the two leads actually had sex on film. But don't count on it unless you've gotten your DVD from the "adult" section.
