Go down to your local Social Security Administration office with your marriage certificate (or divorce papers...depending on what you are doing), and they will have you fill out a paper and turn in your old SS card. You will receive your new one in the mail in a couple of weeks.

You have to have court papers to prove you either changed your name by choice, divorce, or byother reasons. Submit this to SS Admn., and they should send you a new card with the same number, but with the name you desire.

It depends on your state, as a name change is something done at the state level.
You then change all your documents after your local court approves your name change.
That could be a legal name change, or the presentation of a marriage certificate indicating you are changing your name.
