I never heard of Smudger as a nickname for Smith, but in the military it is a common practice to call a Smith by the nickname of Smitty.

You could make it double barrel with Lily to make it Lily-Rose or Lily-Mae, that'd be more unique. You could also use Lilian with Lily as a nickname. I don't have many suggestions with James as men don't tend to have double names. I think the 'always' thing is going too far. Every Smith I've ever known is nicknamed 'Smiffy' or 'Smithy'.
