Without going back and rereading the whole trilogy, I think it was 4 or 5 times. This is from memory, so don't take it for Gospel! Actually it might be 4 because Sam wore the ring once!

I count four - at Bree, at Weathertop, at the breaking of the fellowship, and at Mount Doom. There were lots of times when he almost put it on, but I think these are the only times he actually did!
