Main way? Sharp thing cuts limb off. Har har...

seriously, though, it depends where in the world you are talking about. In war-torn countries, unremoved land-mines cause hellish injuries, and then amputations.
In westernized countries, disease is a major cause. Diabetes, cancers, etc. Also traumatic injuries, such as from industrial and vehicle accidents.

Try entering amputations/accidental in the search and see what comes up.
You really don't give enough info on what you're looking for to get a good answer.
Are you talking about accidental amputations or the docs taking the limb?

Farmers get caught up in the PTO's on their tractors. People that operate heavy machinery. People at printing presses. I believe those are all very common. Then there are the puinishment tyoes of amputations for theft and so that are used in the middle east. There are really to many ways to get any of your body parts amputated to list.

In the movie "Dances With Wolves", if I recall correctly, a guy had his leg amputated by having it sawed off, as you would saw off a block of wood with a hand saw.
