Around 15 to 20 years ago McDonalds began to target coloreds with their marketing. I guess someone in McDonald's marketing department thought mickey d's sounded like "urban" slang for McDonalds and they began to use it in their advertisements. A lot of colored people and white trash then picked up on this phrase and have been using it ever since, even though it makes no sense as you pointed out in your question.

I remember people calling the company "Micky D's" back in the 70s as slang (one of the more polite ones...). I think they finally picked it up and made a series of TV ads with it.

The reason I call it Mickey D's is because my mom and dad used to call it Mickey D's. I suppose it's a faster way to say it!

I think its just a slang term. like j in the b (Jack in the Box/Crack), cjs (Carl's Jr.) or taco hell. Personal favorite= Burger Sh*t (King).
