Go to google and type in myspace applications and it'll bring up a bunch of sites and just pick one, on the menu bar usually to the left side of the page you'll have options. Pick contact box and it'll give you a number of them to choose from, choose one, copy the code and past it in your "about me section of your profile".

search on google for contact boxes myspace, and that should give you some websites with a good variety. you copy the code of the one you like and put it under your about me section.
