I've been thinking of this and I honestly can't think of ONE word. This isn't a negative thing, though. Everyone wants to be considered attractive but a cultural bias leaves us without an acceptable way to express it. Look at the responses you got. Do you think it isn't important to them?
It isn't so much a need to be attractive as a desire. To be proud of one's image isn't negative. To bolster one's self image is a confidence building measure, and outward appearance matters. To care for one's appearance and put the best foot forward. Enhance your appeal to others; make a good (first?) impression. A desire to captivate, intrigue, be pleasing to the eye, have appeal. It isn't just the appearance, it's how the appearance affects how others behave toward us. There's nothing trivial or negative about that.

Vain or vanity is the closest I can think of. Both have the connotation of egotistical, unfortunately.

Well... that's a toughie.
How 'bout an "itch" for approval? Or "emotional hunger" or "compulsion"? Maybe you could use the language of thirsting -- or yearning.
None of these is quite what you're looking for -- but I hope it helps.
