No "perfectly honest" could precede a blunt politically incorrerct answer, it may be used to soften a person before stating the ugly truth which may hurt the person's feelings, so it's a way to warn you before the hurtful words come forth. There's a whole host of reasons some may use this expression but it doesn't mean that they were lying before. For example someone may say: "Do you like my new car?" and you answer well it's okay. which is a way to say I'm not crazy about your new car but I'm not going to make a big deal about it. OR you could answer the same question this way "To be perfectly honest, I'm not crazy about it", which implies a little less enthusiasm

Not necessarily. I think that is just a common expression and not to be taken as meaning they were lying previously.
