With perfect use (using correctly and consistently every day) most forms of birth control are between 97-99.9% effective.

With typical use (only SOMETIMES using correctly and consistently) the rate of efficacy drops.

The rate of efficacy depends on the method of contraception used (some methods are more effective), and whether it is used perfectly or typically. Even if used perfectly, there is that small percentage that a pregnancy could result. That is why no form of contraception is 100%.

There is no such thing as a 100% birth control. The only thing that is 100% is not having sex (but who wants that).

It really depends on what type of birth control your wife is on:
Pill about 95% effective
Condom (Male) about 90% effective
Condom (Female) about 85% effective
Spermicides about 80% effective
Sponge about 80% effective
Diaphragm about 80% effective
Withdrawal about 80% effective
IUD about 98% effective
Injections about 99% effective
Implants about 99% effective
Female Sterilization about 99.9% effective
Male Sterilization about 99.9% effective
Periodic Abstinence about 75% effective

Anything is possible. Nothing is full proof. If she is on the pill you may want to use a condom also. If you're not ready for a baby it doesn't hurt to be to careful.
Good luck!

There is always a chance, But as long as she takes them faithfully then she should be fine. If your really worried wear a condom as well.

if she is on the pill and she takes it faithfully then her chances of getting pregnant are very slim, like an ungodly low percentage.

99.9% it works.. and yes she can still get pregnant. .1% high enough to worry about if your not ready for a baby.
