These are all social programs. That is, programs owned and operated by the community or government. Many are run much better than they would be if they are privatized.
Because of efficiencies of scale and in social programs, the larger the pool of payers as a ratio of the beneficiaries, the cheaper it can be per member.

Problem with your question is there no real definition of "well run". In order to determine what that means, there would need to be some commonly agreed upon measures and metrics.
That said, some that can be said run ok are:
- FAA - air traffic contollers do a pretty good job
- Military Transport wing, Coast Guard, Most military services operate fairly well-- albeit overly expensive.
- Sec. Svc - They seem to be pretty well run and protect presidents
- While I haven't researched this, I've heard that Medicare runs fairly well.
You are naive to think that any entity- government or private is perfect-- they're not. And besides, some things should not be privatized as they truely are public services.
I don't think any **reasonable** person thinks the GOV would run health care. The intent on that is really to provide decent insurance rates to those who need them. If such a thing were to happen, it is clear that insurrance companies would need to administer benefits (medicare part B does that). Also, no plan I've heard of suggest that we "nationalize" all doctors and care-givers. We've seen that it doesn't work in many countries-Canada, European countries etc.
Bottom line is we need to address the root causes of health care increasing costs. Nobody is doing that. We also need some plans so that small businesses and individuals can afford health care. There are reasonable ways of doing it w/out completely socializing medicine. Problem is, so many people aren't even willing to discuss options w/out accusatory comments of going to the extreme.
Let me guess, you have health care via your employer-- do you realize that it is awefully expensive?

Sadly......no there doesn't seem to be too many. We have FEMA trailers rotting instead of being lived in. There's illegals collecting welfare that was intended for our poor. We have illegal aliens employed in airports when our national security is supposed to be checking that. I swear the citizen passengers go through a more through screening to go on 1 flight, than the employees who clean and move the luggage on hundreds. Medicare worked for awhile till the government kept robbing Peter to pay Paul and neglected to put the money back. No...right now I can't think of any program that is working for our citizens.

Government programs just like non-profit organization aren't designed to be efficient.
If the non-profit organization gets close to making a profit the salaries at the top goes up.

I'm unsure of the first question. I would bet private companies could do better, though.
Second question. No. I do not want the government to make decisions about who gets treatment and which type.
