I think you did some of the tips which usually help your system speed up. However, I am posting all the tips and I'd recommend you to redo all the steps, and see the result:

1. Run Disk clean-up. Then delete unused files. (start>program>accessories>sys... tools> disk clean-up)
2. Check for disk errors. (right click on a disk-drive>properties>tools >error checking, then restart)
3. Run disk defragmenter for all drives.
4. Delete cookies. (you can do it from your browser options: tools>internet options)
5. Delete cache or temporary files. (you can do it from your browser options) or, click on 'start", 'run', type 'temp' then click ok. Whatever shows up, just delete.
6. Delete all history files. (you can do it from your browser options, or, CTRL + H, then delete)
7. Empty recycle bin.
8. Delete unused icons & short-cuts from anywhere including desktop.
9. Uninstall and delete any software or game that you use only once or twice in a week, or don't even use.
10. Delete temporary internet files. (you can do it from your browser options: tools>internet options)
11. Delete all recent files. (click on start, click run, then type "recent", delete all the files which show up)
12. Run a full virus scan. If any threat found, eliminate that completely.
13. Run a software which can remove any spyware or adware.
Like: Ad Aware, Windows defender etc.
14. Click Start > Run and type "msconfig" at the prompt. Click the Startup tab and look at all that junk that loads when you launch your PC. Turn off anything else that looks useless, but be careful not to disable Windows system components.

If you did these all at the same time, restart your computer. All these to be done once in a week if possible, depending on the frequency of use of your PC.

* Updating: (Needed only when updates are available)
15. Update your Windows by installing latest updates.
16. Update your audio & video drivers.
17. Update softwares, browsers, games that you use.

If you think your graphics card is weak, or old, you should get a new one, i.e. Geforce 7 series. You should go to http://www.systemrequirementslab.com/referrer/srtest
and check for some new game releases whether your PC is able to handle the games. Check for these new games:

1. Battlefield 2142
2. FEAR Combat
3. Neverwinter Nights 2

If you are playing offline games, stop all background programs like anti-virus, anti-spyware, firewall, Winpatrol etc. Reduce the resolution of your games and remove anti-aliasing. Ensure that you have atleast 20% free sapce in you C: drive and the drive where you have installed your games and your OS. Ensure adequate cooling within the cabinet as heat problems can cause reduction of performance. You problems will not get resolved in totality but you will definately see improvemnet in performance. If you wamnt to see best performance of your PC while palying games, install an OC in a different partiotion for dual boot. It can be WInXP in different partitions of the same hard disk. Use this new loaded OS partition for playing games only. Donot load any unwarranted applications in it - only the barest minimum like DIrect X, drivers for your hardware etc. Install your games in this partition and run them. You will find the required boost. I faced similar problems as you have and I have resolved it this way. I have even gone to teh extent of creating a nother parittion for the swap file. It works.
