As far as museums go, the Thyssen-Bornemisza has a great modern collection that teenagers would probably find much more interesting than the Prado or the Reina Sofia. The Palacio Real (Royal Palace) has some interesting exhibits besides the tour thru the palace (lots of coats of armor and weaponry if you're into that). On Sunday afternoons, provided the weather's nice, wander through the Parque de Buen Retiro and check all the jugglers and street performers. My only other suggestions are to avoid the McDonald's, find a neighborhood bar/restaurant and enjoy some authentic tapas. If they have a map and go in a group, I'm sure they'll have a great time just wandering the city.
ps. avoid the Teleferico, the cable car that runs up the Casa del Campo, it has some nice views of the city but also a good view of the hookers in the park below. For cool panoramic view of the city, check out the Faro de Moncloa for 360º views.
