This is a sentence fragment that uses a connecting word (because), but there is nothing in front of it to connect. This lack of parallelism creates a sentence fragment. I see your confusion on the correct answer, because I can see it both ways.

When we converse, we often ask questions of why something takes place. The person often answers with a sentence fragment that starts with "because."

While this is normal colloquial speech, it is not proper in formal writing. Here is an example:

Why did Karen not attend school today?

Verbal Response: Because she was sick.

Formal Written Response: Karen did not attend school today because she was sick.

This last respons gives the parallelism that is needed when using a connecting word. I think the best answer to the question would be D - lack of parallelism. Someone may very well correct me on this.
